City of Gem Lake <br />Consultant Services Agreement <br />Electrical Inspector.-- Tokle Inspections <br />This is an agreement entered into the day of. by and <br />between the. City of Gem Lake, 4200 Otter Lake. Road, Gem Lake, Minnesota; hereinafter <br />referred to as the city, and Tokle Inspections Inc:, a corporation organized with an <br />address of 174812.3rd Avenue. N.W.; Coon Rapids, Minnesota and existing under the <br />laws of the state of Minnesota, hereinafter referredto as the Tokle (independent <br />contractor). <br />RECITALS. The City desires to Hire. Tokle for electrical inspection services. Tokle, as <br />an independent contractor, willcontrol the means. and manner of performing its services; <br />will be.paid on a per inspection basis; will furnish its own tools, equipment; office <br />s p a ce, an d support. staff. Tokle acknowledges. that it is an independent contractor and it. <br />is not an employee, agent, or representative of the City: Tokle is acting;. as an <br />independent contractor and not: as an. employee, for all purposes, including payment of <br />Social Security withholdinglax and all other. Federal,. state and local taxes; <br />WHEREAS, the city desires to hire the Consultant to render certain technical and <br />professional assistance in connection with such undertakings of the City. in. regard to <br />City electrical construction within the corporate limits; said services are: <br />1. Electrical inspection services based on. Minnesota. State. Building Code. <br />TERMS AND CONDITIONS. Now, Therefore, the parties hereto do mutually agree as <br />follows: <br />I. Review of electrical plans, review building plans as necessary, <br />and issue electrical permits: <br />2. Provide all required on -site inspection services in a timely and <br />.professional manner and within 24 hours of the date of receipt of <br />a. request for, inspection. Tokle shall also provide, on a daily basis <br />or at some other .interval as determined appropriate by the. City's <br />Building Official, a notice indicating when electrical inspections <br />have: been completed.. <br />I Retain all pertinent records and copies of permits and <br />correspondence related to each electrical permit and make such <br />records available to the City upon request.
