Sec. 23 - OPERATING PERMIT <br />(a) SSTS RegWring an Operating Permit <br />An Operating. Permit shall be required ❑f all owners of new holding .tanks, Type IV <br />Systems; Typ& V Systems, MSTS, or any other.system deemedby the Department to require <br />operational oversight, Sewage shall. not be discharged to. these systems. until the. -Department <br />certifies .that the system was installed in substantial conformance With the approved plans; <br />receives the .final. record drawings of the system, and a valid Operating Permit is issued to the <br />owner. <br />(b) Permit AppIicadon Requirements <br />(1) Application for an Operating Permit shall be: made on a form .provided by the <br />D epartrne nt. includ ing' <br />a..Owner name; mailing address, telephone, and email.'address <br />b. Construction Permit reference number and date of issue <br />C. Final record drawings of th.e treatment system.. <br />d. Owners of -holding tanks rnust. submita copy of a valid executed monitoring and <br />disposal contract with a 1icerised maintenance business <br />(2) Owncrs.of holding tanks shall provide to the Department. a.copy of a valid monitoring <br />,and disposal contract executed between the owner and a li"nsed maintenance business, which <br />guarantees the removal of the holding tank contents in a timely manner that prevents an illegal <br />discharge. in accordance with.
