(i) Operating. Permits shall be valid for the specific term. stated on the permit: as <br />determined by the Department: <br />(2) An Operating. Permit must.be'renewed .prior to :its expiration. If not renewed, the <br />Department may require the system to be removed. frvin service or operated as a holding :tank <br />until the. permit: is renewed, If not renewed within ninety. (90) calendar days of the expiration <br />date, .th6 City may require. that the system be abandoned.. <br />Permit. <br />(3).Application Aall.be made on.a form. :provided bytho Department including: <br />a Applicant name, mailing address, telephone number, and e-mail: address. <br />b. Reference number of previous owner's.vperating permit. <br />c: Any andall outstanding Compliance Monitoring Reports as. required by the Operating <br />d. Certified treatment system inspection signed and/or sealed by a certified designer; <br />maintenance contractor, or operator at the discretion of the City. <br />e, Any revisions made to the operation and maintenance manual. <br />f Payment of applicationreview fee as determined by the City.. <br />W Amendments to .Existing Permits not Allowed <br />The City may not amend an existing permit to reflect changes in this Ordinance untie the <br />permit term. has expired and isrenewed, Unless. an amendment is necessary .to eliminate an <br />imminent threat to public Health. or safety: <br />(S} Transfers <br />The Operating Permit may. not be transferred.:A new owner shalt.apply for An: Operating <br />Permit.
