CITY OF GEM LAKE <br />NPDES - MS4 SWPPP <br />BMP DESCRIPTION SHEET <br />A. Unique Identification Number: 4-06-R <br />B. Name or Type of BMP: Complaint Response Program <br />C. BMP Description: The City will develop a more formal complaint response program to address water related complaints. The City will use their <br />existing system of responding to calls to the City Clerk on storm water related concerns. The program phone number and process will be noticed <br />in the Annual Community Notice and on the City's web page. Residents of the City will be able to use the call line to report illicit discharges, <br />report construction site erosion or sedimentation concerns and provide comments on the City's SWPPP. <br />D. Minimum Control Measure Addressed: Construction Site Storm Water Controls <br />E. Describe the measurable that will be used to determine success or benefits of this BMP: <br />1. Post call line on website and in Annual Community Notice. <br />2. Track the number of storm water related calls annually. <br />3. Track the number of staff inspections and follow-up actions initiated from calls and complaints received. <br />4. EDUCATION PROGRAM / PUBLIC OUTREACH COMPONENT <br />a. Audience(s): Any resident, contractor, City staff, business or state or public agency conducting land disturbance or construction <br />activities in the City should identify with the basic goal of the MS4 program - to reduce sediment and other material discharges to <br />City waters from construction sites and to seek awareness of erosion control practices, site clean-up requirements and how they work <br />to protect water quality when properly installed and maintained. <br />F. Describe the timeline or implementation schedule for this BMP: <br />1. Currently in place and ongoing. <br />2. By March, 2004 evaluate the feasibility of linking the compliant system to a storm water data base (BMP 6-05-R). <br />G. Person or department in charge of implementing this BMP: <br />I. Frederic C.
