V1. DESCRIPTION OF REQUESTED SERVICES <br /> All proposals must respond to the following required services: <br /> Ability to provide family appropriate amusement machines and token machines - The <br /> RSC is seeping a vendor that can supply and maintain amusement machines that are <br /> appropriate for all ages and are not offensive to any family. <br /> Leasin arrangements- The selected vendor will provide all of the machines and all <br /> required maintenance. The RSC will received an agreed upon commission from each <br /> machine. The response to the RFP should state a proposal for vendor/RSA" revenue <br /> sharing. <br /> Service arranements- The selected vendor will establish a maintenance program and <br /> service response plan that will insure all machines are operable. <br /> VH. QUALIFIED RESPONDER <br /> All respondents should be qualified to provide the specified services, A list of references and <br /> experiences should be included. Vendors without previous experience should not respond. All <br /> respondents must he able to supply evidence of corporate liability insurance and agree to hold <br /> the City of Roseville harmless for their actions as a vendor at the Roseville Skating Center. <br /> VIH, PROPOSAL OUTLINE <br /> The proposal must include the following information in the order listed: <br /> Provide the name, address, fax and telephone number of your company. <br /> Provide the names, titles} responsibilities and phone numbers of staff representative <br /> involved in this proposal. <br /> Provide a list of references ( with phone numbers and addresses) and experiences <br /> detailing your company's ability to provide the required services and products. <br /> Address each of the issues listed section V1 InClUding proposed revenue sharing. <br /> Provide evidence of insurance. <br /> IX.
