The COLLEGE.agrees to provide the following: <br /> Title of Instruction: EMT Refresher Modular <br /> Date (s) of Instruction: January 6, 8, 13, 1 5,20& 22, 2003 <br /> Name of Instructor: Cindy Anderson <br /> Location: Roseville Fire Department Station #1 <br /> 2, CLIENT'S DUTIES. CLIENT agrees to provide the following materials and/or facilities to the <br /> COLLEGE for the provision of the above-enumerated services at no expense to the COLLEGE: <br /> 1 roster of participants <br /> 2, facility <br /> 3 make all contacts for training and services through the college and will not employ the <br /> presenter/instructor directly for additional sessions. <br /> 3 CONSIDERATION AND TERMS OF PAYMENT. CLIENT agrees to pay the COLLEGE the <br /> following amounts for services, facilities and materials (as applicable). COLLEGE shall provide the <br /> CLIENT with an invoice for services on a monthly/project basis and the CLIENT will make payment <br /> within 30 days of the receipt of the invoice. <br /> Cost: two thousand one hundred dollars ($2,100.00) for up to ten (10) participants.
