Exhibit B. <br />Exhibit <br />L01%G TERM CARE <br />SAMPLE BENEFIT PLAN SUMMARI <br />Eligibility for benefits is defined as a Eons of 2 or more Activities of Daily Living (ADL): <br />Bathing <br />Eating <br />> Transferring <br />Dressing <br />> Toile#ing <br />> continence <br />And /Or cognitive Impairment <br />S-A PL- SCHEDULE-OF BENEWS.. <br />Employer Sponsored Ease Plan: <br />90 day wait following the loss of an ADL <br />$2:000 monthly benefit <br />year maximum benefit <br />50 coverage for home health care <br />Cost: Average $S - $10 per employee per month <br />Employee Buy-Up: <br />A buy -up option would be availab[e for the employees to increase coverage <br />beyond the employer- sponsored benefit. The employee would pay 1 0017o of the <br />premium. <br />Note: Long Term care plans con also be set up as 100% voluntary. The employer would <br />not sponsor a base plan and the employee would pay for the entire benefit. Premiums <br />for voluntary plans are typically priced 157o higher than employer sponsored plans. The ti <br />carrier would need a minimum of 15 -20% participation. <br />
