Exhibit C. <br />This is a proposal for a voluntary individual short - terra disability plan that can be <br />made available to employees on a payroll deduction basis. <br />1,11rom;1 <br />$400 to $5,000 <br />(Maximum income replacement is 66 213% of income. Purchased in $'100 <br />amounts from $400.) <br />Benefit Periods <br />Choice of 3, 6, 12 or 24 months. <br />If $3,100 - $5,000 in monthly benefit is selected a 6, 12 or 24 month benefit <br />period with a 30/30 elimination period or higher is available. <br />4IM-1-11 MI-It ` <br />Choice of 0/7, 7171 0/141 14/14, 0/30, 30/30, 60/60F 0/90, +180/180, based on <br />benefit periods selected. (Elimination period means the number of days following <br />a disability before benefits begin. The first number represents accident <br />elimination period/the second number represents sickness.) <br />There are only three health questions. Po li cies will be issued depending on the <br />answers to these questions. There are no medical exams. For coverage above <br />40% of income or for benefits amounts over $3,000 per month, additional health <br />underwriting questions and information regarding other disability income sources <br />are required. <br />There is no integration Frith ether coverages. Benefits are paid regardless of <br />benefits received from other sources. For coverage over 40% of income or for <br />benefit amounts over $3,000 per month, offsetting occurs during the application <br />process. <br />
