This support which may otherwise not <br />be available—uses volunteers and/or paid nursing staff, and includes: <br />Community Support: Volunteers provide, the following services: <br />0,111 Friendly visiting Transportation <br />Telephone reassurance Assisting with paperwork/explaining Medicare <br />Simple hone repairs Socialization (church, dances, dinners) <br />Lawn garden services Matching friends (multigenerational activities) <br />Additionally, volunteers help rovide or arran a for the following services: <br />Meals on Wheels /congregate dining jj Respite support <br />Homemaking/chore service Senior center activities <br />Adult day care Legal/financial assistance <br />Health : Opportunities the Program offers throughout the year include: <br />Community health fairs a workshops on topics such as falls prevention, <br />Individual consultations with the elder/caregiver advance directives and vision impairment <br />on health issues <br />General Health Assistance: Partnering with' a Medicare certified nursing agency, profession- <br />al nursing and trained staff work collaboratively to provide the following services: <br />Health history and assessment Medication management <br />Vision, hearing and depression screening i Referrals /reminders for health maintenance (flu <br />Blood pressure/blood glucose monitoring shots, mammograms etc.) <br />Case Management: The Program Nurse works with the elder and other key individuals to: <br />Identify health problems Consult with other service providers (physicians, <br />Determine needed services therapists, social workers) <br />Identify who will provide such service Evaluate outcomes and adjust care plan when <br />indicated <br />SkIlled Health Care Services: When necessary, support is arranged /coordinated with the <br />partner nursing agency or other community providers. <br />Program offers: Skilled nursing, home health aide support, durable medical equipment <br />Program Refers /Arranges: Physical /Occupational /Speoch therapies, hospice care <br />
