Final Memorandum of Agreement (MOA) SRFC Safe Schools/Healthy Students Initiative (SS/HS) Connections Grant Partners of Suburban Ramsey County is to help SSMS project partners achieve the ob ectives of the proposed program on time j and within budget, including meeting their partnership responsibilities, providing staff, and contracted services, and setting milestones for accomplishing project tasks. The SS/HS Cor Management Team is responsible for providing direction to the SSMS Project Director and for leading and managing SSMS project goals and collaborative processes. Ke administrators from each school district, a local law enforcement representativ juvenile justice and a county mental health services representative will meet quarterly with the SSMS Project Director to guide, facilitate and promote the goals of the SSMS grant. (See attached roster of the Core Management Team) The SS/HS Design Team is responsible for designing-in ongoing project planning activities, training sessions, and providing evaluation implementation support using the Casey P.D.S.A. Change Model of (Plan, Do, Study, Act). Key district managers will meet with the SSMS Project Manager and the SRFC Assistant Director before each SSMS Core Management Team Meeting to give progress reports and address concerns, problems or barriers affecting the implementation of the comprehensive community wide plan and SSMS project goals. This team's questions, updates, concerns or emergent needs will be a standard item on the SSMS Core Management Team's agenda. (See attached roster of the Design Team) eam) At the School Level: • The Superintendents of the four district consortium will be key communicators and coordinators for the day-to-day district-wide SSMS project and partnership involvement. • The School Building, Principals will be key communicators and coordinators for the day-to-day school-wide SSMS project and partnership involvement. • Each school district will hold regular SS/HS Interdisciplinary Team meetings. SSMS Design Team members will facilitate these meetings and, with the team's assistance, insure steady progress on the programlprojectleurriculum and services of the SSMS project and coordinated services. The attached SSMS Logic Model, The Governance Flow Chart, the SAT and SSMS Flow Chart and "Partnership Process of Continuous Improvement" are attached to this MOA for further clarification of the roles, responsibilities and programs of the SSMS grant.
