<br />EXHIBIT "A" <br />04/14/97 <br />PF2882 <br /> <br />Conditions to be attached to Conditional Use Permit Resolution <br /> <br />This conditional use permit (CUP) shall be for one communications tower consisting of a <br />150 foot steel, self supporting telephone tower and equipment cabinets on the <br />southwestern corner of property commonly known as the St. Paul Water Authority <br />Reservoir site with conditions as follows: <br /> <br />1. The maximum height above grade at the tower base shall be 150 feet. Up to12 <br />radio/antenna at a size specified in the application, may be installed on the tower. <br /> <br />2. The applicant shall apply for and receive a license/tower permit from the City of <br />Roseville prior to applying for a building permit. <br /> <br />3. All CUP application materials shall be considered part of the permit. The applicant <br />shall apply for and receive a building permit prior to commencing construction. Supporting <br />documents for the building permit shall include a Minnesota registered engineers <br />calculations and certification of structural strength and stability. <br /> <br />4. Approvals in writing from the Water Authority for access over, under, or on the <br />surface of Water Authority property by machinery technicians, and underground wires shall <br />be included in the building permit application. <br /> <br />5. The base of the tower shall be located in an area at least 150 feet from any <br />occupiable structure. <br /> <br />6. All utility wires or lines leading to the equipment panel site shall be buried. <br /> <br />7. The applicant shall adhere to provisions of City Code Section 1012.10, subp. 5 <br />through 7 and 9 through 12, except that paved driveways and parking spaces shall be at <br />the discretion of the City. <br /> <br />8. The proposed steel tower shall be self supporting - no wires or guys are necessary <br />to anchor it. The cellular phone antennas on the tower (up to twelve - 4 companies/ 3 <br />antennas each) shall be a maximum size of 6.6 feet long, 6 inches wide, and 1.5 feet thick. <br />These antennas will operate in the 1.9 Ghz to 2.2 Ghz radio frequency range, transmitting <br />and receiving a low power wireless radio signal to a service cell primarily in eastern and <br />central Roseville. No sewer or water service shall be installed. <br /> <br />9. A prefabricated set of equipment panels may be installed near the base of the <br />tower. They will be connected to the tower by one cable to each antenna. <br /> <br />10. A 30 foot by 40 foot equipment panel area with security fencing (up to 8 feet high <br />
