<br />(6) "Personal Campaign Committee" means any committee appointed by any <br />candidate for city office for any city election. <br /> <br />(7) "Political Committee" means any two or more persons who cooperate in raising, <br />collecting or disbursing money or contributions in kind to be used for or against the <br />election of any candidate for city office, or for or against voter approval of any city <br />ballot question. <br /> <br />B. Campaign Contribution Disclosure Reports <br /> <br />(1) Every candidate or personal campaign committee, and every political committee <br />shall file the reports required by this section in any year that it receives <br />contributions or makes expenditures of one hundred dollars ($100.00) or more. <br /> <br />(2) Reports required by this section shall be filed with the city elections coordinator: <br />(a) within 14 days after a candidate, personal campaign committee, or <br />political committee receives contributions or makes disbursements of more than <br />$750.00 in a calendar year; <br />(b) 10 days before any primary or special primary election; <br />(c) 10 days before any general election or special election; <br />(d) 30 days after any general or special election; and <br />(e) at the same time as any other campaign finance reports required under <br />Minnesota Statutes Chapter 211A. <br /> <br />(3) Reports required by this section shall be signed under oath and certified as true <br />by the person filing the report. The city elections coordinator shall supply forms for <br />reports required by this section. <br /> <br />(4) Each report made under this section shall disclose: <br />(a) Every contribution of money or other thing of value received by a <br />candidate, personal campaign committee or political committee during the reporting <br />period, together with the name and address of the person or source from which it <br />was received, and the date it was received; <br />(b) Every contribution in kind received by a candidate, personal campaign <br />committee or political committee during the reporting period, together with the <br />name and address of the person or source from which it was received, and the date <br />it was received. Each contribution in kind shall be valued at fair market value. <br /> <br />(5) No anonymous contributions or contributions in kind shall be accepted or <br />retained by a candidate, personal campaign committee or political committee. <br />
