Vaeation Pay: If an employee desires vaeatioR pay in advanee of'laeation, he/she mHst <br />submit tho request in writing at least t';lO (2) weeks prior to the 'laeation date. <br />GE. Vacation Pay on Termination of Employment: f.n employee who terminates his/her <br />employment shall receive, Hpon termination, HnHscd vacation pay. <br />Paid Time Off on Termination of Employment: An employee who terminates his/her <br />employment shall receiye, upon termination, unused Paid Time Off. <br />H. Vacation Time: all yaeation periods of more than fi'le (5) days must be an-anged at least <br />thirty (30) days in advance and the times of such vacations shall be subject to the approval <br />of the department head. Employees will be given opportHnity to selcct 'racation periods <br />insofar as it is practical. 8election of'lacation time shall be based on service credit. Vacation <br />periods of five (5) days or less should be an-anged at least one '.veek in advance. <br />IG. Can-yo'/er and Waiver of Vacation Leave: No employee shall be permitted to cany over <br />vacation time from one year to the next. Provided that, ifin the opinion of the city manager, <br />unusual circumstances exist, he or she may approye a eanyover of all or part of the previoHs <br />year's vacation. <br />Can-yover and Disposition of Paid Time Off: Each employee shall be permitted to can-y over <br />UP to a maximum of 500 hours from one year to the next. Allowable disposition of hours in <br />excess ofthe maximum may be contributed to defen-ed compensation accounts or pre- <br />dedicated for use within the first quarter of the following year. <br />J. 8ick Leave Plan: RegHlar fHll time employees may use sick leave f.or: <br />1. Pemonal illness or injary. <br />2. Pemonal medical or dental appointments, <br />3, Illness or injury of a spoase, child, parent or other member of an empleyee's hoasehold <br />which re~ires the employee's care and attention. <br />4. Medical or dental appointments of a spouse, child, parent or other member of an <br />employee's household which reElaire the employee's care and attention. <br />K. PmofReElHired: Iii order to be eligible for siek leave pay, an employee must: <br />1, Report promptly, to the sapervisor, the reason for the absence, <br />2. Keep the sHpervisor informed ofthe employee's condition if the absence is more than <br />three (3) working days' daration, <br />3. 8ubmit a medical certificate for any absence exceeding three (3) working days, ifreEluired <br />by the city manager or designee, <br />4. 8abmit a medieal eertifieate for absences oness than three (3) working days when the <br />frequency or pattern of sick leave usage indicates possible abuse of sick leave benefits, (Ord. <br />1135,2 11 1991) <br />loR. Rate of Accumulation for 8ick Leave: 8ick leaye pay will be granted to regular employees, <br />aceording to accHmalated earned service credit from the date of employment at the rate of <br />eight (8) hours per month. 8ick leave may be accumulated to an unlimited amount.
