SECTION 3 - STRUCTURE AND CONTENT OF RESPONSE <br /> 3.1 RESPONSIVENESS (Pass/Fail) <br /> • Respondents must prepare submissions that follow the format and sequence <br /> specified in this RFP. This includes adherence to the format of any attachments. <br /> • The following conditions/criteria must be met in order to be considered responsive: <br /> (1) The Respondent will complete and provide all information in Attachment A. <br /> (2) The Respondent will complete and provide all information in Attachment B. <br /> (3) The Respondent will complete and provide any exceptions in Attachment C. <br /> (4) The Respondent will complete and provide all information in Attachment D. <br /> (5) The Respondent will complete and provide all information in Attachment E. <br /> (6) The Respondent will complete and provide all information in Attachment F. <br /> (7) The Respondent will complete and provide all information in Attachment G. <br /> (8) The Respondent will complete and provide all information in Attachment H. <br /> (9) The Respondent will complete and provide all information in Attachment I. <br /> 3.2 FORMATTING REQUIREMENTS <br /> • The formatting requirements apply to the following Attachments. Any additional <br /> formatting requirements will be identified each Attachment's description. <br /> • Attachment E — Risk Plan <br /> • Attachment F — Project Capability Plan <br /> • Attachment G —Value Add Plan <br /> • In order to minimize any bias, the attachments identified above must NOT contain <br /> any names that can be used to identify who the Respondent is (such as company <br /> names, personnel names, project names, product names, etc.). A template for each <br /> Attachment is provided in this document and must be used by all Respondents. <br /> Respondents are NOT allowed to re-create, re-format, or modify the template <br /> (cannot alter font size, font type, font color; add colors, pictures, diagrams, etc.). An <br /> electronic copy of this document is available for download and must be used. Each <br /> individual attachment must NOT exceed two (2) pages and should be completed <br /> only on the front side of each page. <br /> 3.3 PROPOSAL INFORMATION <br /> ■ The Respondent will prepare and submit a proposal form (Attachment D) <br /> ■ All required information must be filled out <br /> 3.4 RISK PLAN <br /> • The Respondent must submit Attachment E. The Respondent should list and <br /> prioritize major risk items on this project could cause the Respondent's "vision" or or <br /> "plan" to deviate or not meet the expectations of the client (i.e. risks that the <br /> Respondent does not control). This includes sources, causes or actions that are <br /> beyond the scope of the contract that may cause cost increases, delays, change <br /> Page 21 of 40 <br />
