Subject: Short Term Disability (STD) Reference #: s.�o <br />Scope: Regular PT/FT City Staff on PTO Program <br />Authorizing Authority: <br />Effective Date: �iiio6 Revision Dates: Page 1 of 6 <br />PURPOSE: <br />The purpose of this policy is to provide written guidelines of the eligibility, benefit, and <br />administration of the City of Roseville's Short Term Disability (STD) policy. <br />POLICY: <br />The City of Roseville provides regular full-time and part-time benefit accruing employees who <br />participate in the Paid Time Off (PTO) Program, as supplement to the program, a short-term <br />disability benefit plan. No enrollment forms need to be completed. Employees are automatically <br />enrolled once they are eligible. This benefit is provided through a self-funded City account and <br />there is no cost to the employee. <br />If, due to a disability, an employee is absent from work for more than 14 consecutive work days <br />the City will continue the employee's salary at 80 % from the 15th calendar day through the 63rd <br />calendar day (based on an eight hour work day, pro-rated for part time) of your absence. <br />The employee must provide documentation from a physician, approved by the City, certifying <br />that the employee cannot return to work in some capacity the City finds acceptable. The City <br />Manager or designee is responsible for administration and program guidelines for STD. The City <br />reserves the right to modify or terminate the program in its discretion. <br />ELIGIBILITY: <br />In order to qualify for the STD program, employees must be participating in the PTO program and <br />must have successfully passed the probationary period. The employee must also be a regular <br />benefit accruing and actively at work member of the staff on the effective date of coverage in order <br />to qualify for coverage at that time. Short term disability is available only for an employee's non- <br />work related disability. It may not be used to care for ill or injured family members. <br />DEFINITION OF A DISABILITY: <br />A disability is defined as a physician certified non-work related injury (an accidental bodily <br />injury) or illness (a physical or mental illness) including pregnancy that precludes the employee <br />from performing the duties of their job description in some capacity the City finds acceptable. A <br />determination that an employee is able to work a modified schedule (i.e., part-time) would NOT <br />disqualify him/her from receiving benefits. <br />� � <br />
