3. The area surrounding the equipment shelter and monopole must be screened with either a fence and/or landscaping. If landscaping is selected, plant material must be installed on the south, west, east sides of monopole/equipment area at a minimum height of five feet (except accent shrubs); 75% of this plant screen must be coniferous. A complete landscape plan must be prepared as part of the building permit application and approved by the City prior to issuance of the building permit. 4. The monopole is to be limited to the single user/applicant (Qwest Wireless). Future antenna additions will require a revised CUP. 5. The monopole must not be lighted. 6. Electric and phone wires serving the equipment and monopole must be buried. 7. Parking of maintenance vehicles must occur in the off-street parking space directly adjacent to Lovell Avenue. 8. The 60 foot tall monopole and supporting equipment must be completely removed from the site by the applicant (Qwest Wireless) after operations cease; the equipment is damaged or unsafe as determined by the Roseville Building Official; the CUP elapses or is determined to be in violation by the Community Development Director. The City may require the applicant (Qwest Wireless) to post a bond to assure timely removal ofthe monopole and supporting equipment. 9. The applicant must provide the Community Development Department with the legal description of the property allowing proper recording of the CUP resolution. 10. Prior to issuance of a building permit, and within 60 days after approval of the conditional use permit must be recorded with the Ramsey County Recorder. 11. This conditional use permit shall expire 18 months after approval if a building permit has not been issued. (Section 1013.03) NOW THEREFORE BE IT RESOLVED, by the Roseville City Council, to approve granting Qwest Wireless a conditional use permit to allow the installation of a wireless communication device (monopole) and an associated equipment cabinets 2335 Dale Street, based on findings and conditions indicated above. The motion for the adoption of the foregoing resolution was duly seconded by Council Member Mastel, and upon vote being taken thereon, the following voted in favor: and the following voted against the same: None WHEREUPON said resolution was declared duly passed and adopted. 3
