<br />Air Emission Pennit No. 12300387-9 <br />Page 13 of 22 <br /> <br />The Permittee, in the event of any other type of deviation from permit conditions, including <br />those attributable to upset conditions, is to report the deviation within two working days and <br />provide the above information listed in the above paragraph. Unless stated otherwise in the <br />permit, a report filed under this provision does not satisfy the requirement to notify the <br />MPCA of shutdowns and breakdowns under Minn. R. 7019.1000. <br /> <br />Note: Upset condition is defined as a variation in operating conditions which causes <br />monitored parameters to exceed the limits specified in the permit, but which is not a <br />shutdown or breakdown. <br /> <br />· Monitoring Equipment. In Accordance with Minn. R. 7019.1000, subp. 4, the Permittee <br />shall notify the Commissioner or designee of any breakdown or malfunction of any <br />continuous monitoring system or monitoring device. Notification shall be made within <br />72 hours of the breakdown or malfunction and shall include an estimate of the duration of the <br />monitor downtime. While the primary monitoring method is down, if an alternative <br />monitoring method is available, the notification shall also identify this method, and the date <br />and time that the use of the alternative monitoring method commenced. The Permittee shall <br />also make a second notification when the breakdown is over. Notifications shall be made in <br />accordance with this section. <br /> <br />7.3 General Recordkeeping Requirements <br /> <br />The Permittee is required by previous parts of this permit to record and submit to the AQD <br />reports and/or other documents listed in this permit according to the method and schedules <br />identified in Part 6 of this permit. <br /> <br />The Permittee shall retain records of all recording/monitoring data and support information for a <br />period of five years, or longer if specified elsewhere in this permit, from the date of the <br />monitoring sample, measurement, or report. Support information includes all calibration and <br />maintenance records and all original strip-chart recordings for continuous monitoring <br />instrumentation, and copies of all reports required by the permit. Records shall be kept at the <br />stationary source unless the permit allows otherwise. <br /> <br />The Permittee shall maintain records adequate to document the compliance status of the <br />stationary source, including at a minimum: <br /> <br />1. The date, place, as defmed in the permit, and time of sampling or measurements; <br /> <br />2. The date or dates analyses were performed; <br /> <br />3. The company or entity that performed the analyses; <br /> <br />4. The analytical techniques or methods used; <br /> <br />5. The results of such analyses; and <br /> <br />6. The operating conditions existing at the time of sampling or measurement. <br />
