Attachment "A" 04/14/97 PF 2882 Conditions to be attached to be attached to Conditional Use Permit Resolution. This conditional use permit (CUP) shall be for one communications tower consisting of a 150 foot steel, self supporting telephone tower and equipment cabinets on the southwestern corner of property commonly known as the St. Paul Water Authority - Reservoir site with conditions as follows: 1. The maximum height above grade at the tower base shall be 150 feet. Up to 9 radio/antenna at a size specified in the application, may be installed on the tower. 2. The applicant shall apply for and receive a license/tower permit from the City of Roseville prior to applying for a building permit. 3. All CUP application materials shall be considered part of the permit. The applicant shall apply for and receive a building permit prior to commencing construction. Supporting documents for the building permit shall include a Minnesota registered engineers calculations and certification of structural strength and stability. 4. Approvals in writing from the Water Authority for access over, under, or on the surface of Water Authority property by machinery technicians, and underground wires shall be included in the building permit application. 5. The base of the tower shall be located in an area at least 150 feet from any occupiable structure. 6. All utility wires or lines leading to the equipment panel site shall be buried. 7. The applicant shall adhere to provisions of City Code Section 1012.10, subp. 5 through 7 and 9 through 12, except that paved driveways and parking spaces shall be at the discretion of the City. 8. The proposed steel tower shall be self supporting - no wires or guys are necessary to anchor it. The cellular phone antennas on the tower (up to nine) shall be a 5
