hearing regarding the Qwest Wireless request and recommended (7-0) approval ofthe conditional use permit, subject to the following conditions: 1. The maximum height above grade at the monopole base may be up to 60 feet. One antenna cylinder of the size and pole location specified in the application may be installed on the monopole. 2. The equipment cabinets must be erected adjacent to the monopole in the location specified in the application and must not exceed 10 feet by 20 feet or 200 square feet in size and shall not exceed 10 feet in height. All utility wires or lines leading to the monopole from the equipment shelter must be buried. 3. The area surrounding the equipment shelter and monopole must be screened with either a fence and/or landscaping. If landscaping is selected, plant material must be installed on the south, west, east sides of monopole/equipment area at a minimum height of five feet (except accent shrubs); 75% of this plant screen must be coniferous. A complete landscape plan must be prepared as part ofthe building permit application and approved by the City prior to issuance of the building permit. 4. The monopole is to be limited to the single user/applicant (Qwest Wireless). Future antenna additions will require a revised CUP. 5. The monopole must not be lighted. 6. Electric and phone wires serving the equipment and monopole must be buried. 7. Parking of maintenance vehicles must occur in the off-street parking space directly adjacent to Lovell A venue. 8. The 60 foot tall monopole and supporting equipment must be completely removed from the site by the applicant (Qwest Wireless) after operations cease; the equipment is damaged or unsafe as determined by the Roseville Building Official; the CUP elapses or is determined to be in violation by the Community Development Director. The City may require the applicant (Qwest Wireless) to post a bond to assure timely removal of the monopole and supporting equipment. 9. The applicant must provide the Community Development Department with the legal description of the property allowing proper recording of the CUP resolution. 10. Prior to issuance of a building permit, and within 60 days after approval of the conditional use permit must be recorded with the Ramsey County Recorder. 11. This conditional use permit shall expire 18 months after approval if a building permit has not been issued. (Section 1013.03) 3
