Paschke. Thomas

From:
Sent:
To:
Subject:

Christine Baker [Christine.Baker@state.mn.us]
Thursday, June 21, 2001 11 :30 AM
thomas.paschke@ci.roseville.mn.us
Re: Forest Lawn Cemetery

Dear Tom,

Inspections are done at time of installation then we re-inspect biennually, and all the
crematories will be revisited this August. As far as the state goes for emmissions
testing, there are no guidelines. There could be federal EPA guidelines, but we don't
know what those would be. The best way to find that information out would be to contact
IEE (Industrial Engineering and Equipment) 1.800.327.2831. It's a company that builds the
majority of the crematories for the U.S. and Forest Lawn has one of their crematories
(actually most crematories in our state were purchased from there). The company also
should have reports on their research and development of crematories.

When we inspect crematories here's what we look for [followed by MN statue-rule] :
1. Enviromental license (if required) [149A.95 Subd.2]
2. Employee equipment operation training [149A.95 Subd. 3]
3. Universal precautions [149A.95 Subd. 3 & 7]
4. Written authorization form [149A.95 Subd. 4]
5. Proper encasement of remains [149A.95 Subd. 6]
6. Proper permits/authorizations [149A.95 Subd. 6]
7. ID of remains procedure [149A.95 Subd. 8]
8. Privacy [149A.95 Subd. 10]
9. Proper processing {149A.95 Subd. 14]
10. Cremated Remains Container [149A.95 Subd. 15]
11. Accuumulated residue [149A.95 Subd. 17]
12. Release of cremated remains [149A.95 Subd. 18]
13. Record Keeping [149A.95 Subd. 20]
14. Display of License [149A.52 Subd. 5]
15. Holding Facility [149A.52 Subd. 2]

I hope this answers your questions. If you need any additional information regarding this
matter, please feel free to email me.
Christine Baker

Christine Baker
Program Analyst/Mortician Investigator
Mortuary Science
121 East 7th Place, Suite 400
St. Paul, MN 55164-0975
Phone: 651.282.6350
Main: 651.282.3829
Fax: 651.282.3839
Cell: 612.986.1622
email: christine.baker@health.state.mn.us

>>> <thomas.paschke@ci.roseville.mn.us> 06/21/01 09:58AM >>>

Ms. Baker:

I am the City Planner for the City of Roseville seeking information
regarding the crematory at Forest Lawn Cemetery.
