<br />withstanding the fact that the organization does not maintain <br />an address within the City. <br /> <br />2. No organization may conduct lawful gambling at more than <br />three (3) locations within the City. <br /> <br />3. A. The maximum number of Bingo Hall licenses and locations <br />within the City shall be one (1). Once the Bingo Hall <br />license is issued by the City, it shall be limited to the <br />location and to the owner specified on the license. Any <br />change of location or ownership without the approval of <br />the City shall result in the termination of the license. <br /> <br />B. The maximum number of Class A premises permits for that <br />location shall be seven (7). All Class A premises <br />permits and Bingo Hall licenses shall be operated <br />independently of any and all other Class A, B or C <br />licenses issued in the State of Minnesota. The Class A <br />permits shall terminate simultaneously with the <br />termination of the Bingo Hall license. <br /> <br />C. The maximum number of Class B premises permits in the <br />City shall be eight (8). <br /> <br />D. There shall be no Class C premises permits in the City. <br /> <br />119.020. Approval of Bingo Hall License and Premises Permits. <br /> <br />1. Any organization applying to the Gambling Control Board for a <br />Premises Permit, Bingo Hall License, or for the renewal of <br />the same, to conduct lawful gambling in the City shall, <br />within ten (10) days of making such application, file the <br />following with the City: <br /> <br />A. A duplicate copy of the Gambling Control Board <br />application along with all supporting documents submitted <br />to the Gambling Control Board. <br /> <br />B. A copy of the Articles of Incorporation and Bylaws of the <br />organization. <br /> <br />C. The names and addresses of all officers and directors of <br />the organization. <br /> <br />D. A copy of the organization's written procedures and/or <br />criteria for distribution of funds derived from lawful <br />gambling, its standardized application form, and its <br />written fiscal control procedures. <br /> <br />E. A copy of the Internal Revenue Service's tax exempt <br />letter. <br />
