Attachment A <br />RESPONDENT OFFER — SIGNATURE AND CERTIFICATION FORM <br />The undersigned has carefully examined all instructions, requirements, specifications, terms and <br />conditions of this RFP; understands all instructions, requirements, specifications, terms and conditions <br />of this RFP; and hereby offers and proposes to furnish the services described herein at the prices quoted <br />in the Respondent's Proposal, and in accordance with the requirements, specifications, terms and <br />conditions of this RFP. <br />The Respondent also certifies: <br />I.
