• Behavior such as fighting, practical jokes, or throwing articles at each other will not be tolerated. <br />• Fire extinguishers and fire exits are not to be blocked by supplies, debris, or equipment at any time. <br />• Perform only those jobs you have been assigned and properly instructed to perform. <br />• Keep all mechanical safeguards in position during operation. <br />• Follow Lockout/Tag Out procedures whenever making adjustments, when setting up jobs or when <br />machine is to remain idle for any length of time. <br />• Compressed air should not be used for cleaning clothes, cooling or practical jokes. <br />• Employees must ask permission to use company tools or equipment for personal use. <br />• Maintain at least 10 feet distance from people and inbound recycling trucks while driving rolling stock <br />(fork lifts, Front End loaders, etc) <br />Employees who violate these safety guidelines may be subject to disciplinary action. <br />Accident/ Injury Investigation <br />If an accident or near miss takes place and there is no attempt to identify the causes and take preventive <br />action, then it is not a question of "if', but "when" the situation could happen again. <br />All incidents, accidents and injuries are to be reported to your supervisor immediately. If an accident <br />occurs, we will follow these steps: <br />• The employees immediately notifies supervisor. <br />• The supervisor will be responsible for completing an investigation of the accident. Most of our yard <br />and our tip floor are monitored by cameras that can be used for investigative purposes. The <br />investigation will be documented using the Accident Investigation Report form. The purpose of the <br />investigation is fact finding— not fault finding. If possible, the supervisor will immediately correct any <br />apparent hazards and document these changes. <br />• The supervisor will return the completed Accident Investigation Report in to the Human Resources <br />office for review by the end of the workday. Management will determine corrective actions in order <br />to avoid similar future accidents. <br />• The Human Resources Department will complete a First Report of Injury form if medical treatment is <br />necessary. Our Workers Compensation carrier will be notified of the claim. <br />• HR and Management will cooperate with the recommendations of our Workers Comp carrier and the <br />treating doctor. <br />• If applicable, Human Resources will ensure that the injury is recorded on our OSHA 300 Log. <br />• If there is an accident that results in an employee with restrictions, we will try to get that employee <br />back to work as soon as possible. Any limitations will be accommodated to the best of our ability. <br />New Employee Orientation and Ongoing Safety Training: <br />Each new and existing employee will be trained on Eureka Recycling's Safety policies as they apply to the <br />employee. Training will be given at the beginning of employment, and also on an ongoing basis during <br />employment as it is appropriate and required. <br />The following categories are included in Eureka Recycling's Safety Handbook with accompanied Safety <br />Training where applicable: <br />• AWAIR <br />• Accident/Incident Prevention <br />• Emergency Procedures <br />
