City of Roseville <br />RFP for Recycling Collection Services <br />Proposal Checklist <br />Respondents must prepare submissions that follow the format,* attachment titles, and sequence specified in this <br />REP. <br />Respondents must complete and submit this checklist* of items for inclusion in the proposal. Each item must be <br />submitted on a separate page. <br />Responsiveness (Pass/Fail) <br />The Respondent must submit each of the following documents in order to be considered responsive: <br />R Completed Proposal Submittal Form and Affidavit of Non -Collusion (Attachment C).* <br />R Statement as to any litigation in the past five years within the State of Minnesota and the current status <br />of that litigation. <br />10 List of firm's principal officers' names, titles, addresses and contact information (telephone and email) <br />for designated contact person. <br />10 A comprehensive cart rollout plan and schedule for the initial cart `distribution, including cart switch <br />outs if a new Contractor replaces the current contractor. <br />10 Examples of monthly and annual reports. <br />10 Example of the Respondent's annual report and educational materials sent to multi -unit building owners. <br />10 A statement clearly specifying the location(s) of its recyclables Materials Recovery Facility (MRF) or <br />subcontractor's MRF) where material collected from the City will be delivered and / or processed. If a <br />third party MRF is used, a letter of agreement or understanding from the MRF subcontractor. <br />10 Letter certifying all recyclables to be collected in the City's recycling program will have adequate end <br />markets. The letter should include a statement on the Respondent's proposed end market locations as a <br />strategy to help sustain the city's Recycling program for the long-term. <br />10 Examples of Respondent's public education materials including education tags used during curbside <br />collection operations if unacceptable recyclables are rejected by collection crews. <br />10 Example of Respondents monthly unacceptable recyclables reporting form. <br />10 List of references. <br />10 Respondent Questionnaire (Attachment D).* <br />RFP for City of Roseville Recycling Services <br />
