Eureka has a safety committee that includes managers and front line staff across all departments. <br />The safety committee meets on a monthly basis to identify and implement proactive safety <br />measures and to review any situation where an injury has occurred as well as any safety issues <br />and concerns raised by staff. A risk management consultant from our workers compensation <br />insurance company attends our safety committee meetings to provide us with real time support <br />and suggestions towards continuously improving our safety practices. <br />Summarize your safety plans for MRF operations to protect workers and customers when <br />in the plant. <br />Eureka Recycling utilizes the practice of employing full-time and fully benefited employees in <br />our MRF, which has limited the turnover of our staff. This allows for a fully trained and safety - <br />aware staff to be operating the equipment and on the sorting lines in the facility at all times. The <br />safety program described above includes all Eureka Recycling employees including our team in <br />the MRF. Eureka Recycling has experienced very little loss -time accidents in our MRF <br />operations since we opened in 2003, exceeding industry standards. <br />Only pre -approved customers are permitted to bring recyclable material to Eureka Recycling. All <br />customers are provided with a copy of scale policies and a site map that provide safety <br />instructions including appropriate traffic speeds, traffic flow and how to tip/empty their trucks in <br />a safe manner (including required PPE for the tip floor and safe distance required between <br />vehicles). No more than four trucks are permitted to dump at one time and traffic to dumping <br />space is directed by equipment operators on the tip floor to ensure the floor is safe before they <br />dump. Equipment operators are instructed to leave at least 10 feet of space between their vehicles <br />and other vehicles on the tip floor and report any unsafe behavior seen with either inbound trucks <br />or other Eureka Recycling staff. <br />Equipment operators all have a two-way radio they can use to alert scale staff and management <br />staff in case of an emergency. Cameras on the tip floor allow managers to see what is happening <br />in real time and review history in case there is need to investigate an incident. Cameras are <br />regularly observed to ensure safe practices by both our staff and other parties. <br />We have invested in significant training of our MRF managers and supervisors to identify <br />hazards and near misses, and we review these both in daily MRF manager meetings and in <br />monthly Safety Committee meetings in order to maintain an intense proactive focus on safety in <br />the MRF. <br />Have you had any OSHA — reportable accidents within the past three years in Minnesota? <br />If so, summarize the individual events, and corrective actions taken. <br />Yes - we did have one OSHA reportable injury within the past three years. It occurred on <br />7/23/2019, though the employee declined treatment at the time of the injury and did not seek <br />medical care until 7/25/2019. The injury was the result of an employee tripping on uneven <br />ground and hitting their head and body against the back of a car and the ground, causing some <br />internal bruising and broken bones. This employee was admitted to the hospital on an inpatient <br />basis, which is the reason we reported it to OSHA. Since then we have completed an assessment <br />of the site where the incident occurred and taken steps to direct foot traffic away from that area, <br />including removing the smoking table (became a non-smoking worksite) and adding signs <br />announcing uneven ground where there is landscaping to promote water drainage. <br />RFP for City of Roseville Recycling Services • D-7 <br />
