Documentation submitted to MCES at project completion to verify eligibility and calculate both PMA and FRA must <br />include the following: <br />• Completion and submission of MCES provided cost verification form <br />• A city resolution authorizing participation in the grant program <br />• Certification (notarized) confirming ownership or easements for locations where work was completed <br />• Description of work, along with description or map of locations <br />• Invoices substantiating cost of work completed. <br />This letter is a commitment to enter into a legally binding grant agreement upon verification that grant program <br />guidelines and requirements have been met.
