Hickok & Associates <br />PE T RCWD File <br />Permit Application No. 88-167 <br />..Issued to: Frank Rekuski, Jr., 1225 Josephine Road, Roseville, MN 55113' <br />Location: 1225 JosephineRoad, Roseville. <br />Purpose: Approval of "after -the -fact" Shoreline Improvement for construction <br />of retaining wall and building additions riparian to Lake Josephine. <br />At their meeting on -November 23, 1988 , the Board of Managers of the Rice Creek Watershed <br />District reviewed your permit application and the recommendation of the District Engineer <br />based upon the following exhibits: <br />1.. Permit Application dated October 12, 1988. <br />2. Plan view and cross-section drawings of project site, received October 13, 1988. <br />They found the project as planned to be in accordance with the Rules & Regulations and Guide- <br />lines of the District and therefore approved the permit as requested, with the following ad- <br />ditional stipulations: No additional stipulations. <br />NOTE:. In order to comply with MN/DNR shoreland ordinances, this approval is granted <br />with the understanding that the proposed groundlevel dick will be installed <br />without railings. (Addition of railings would require the deck to conform to <br />setback distances from Josephine Lake.) <br />It is the responsibility of the Pernuttee to provide all measures necessary to positively <br />contain sediment on the site during construction. This may require the use of erosion <br />control measures not outlined on the erosion control plan. If this requirement is not met, <br />the Permittee may be ordered to provide additional erosion control measures if deemed neces- <br />sary by the District Inspector. It is also the responsibility of the pmperty owner to pro- <br />vide whatever maintenanoe is necessary to assure that the stormwater management systems ap- <br />proved as part of this permit will continue to function as originally designed. <br />A followup inspection will be conducted at the project site to assure oonpliance with all <br />agreements, terns and/or stipulations to this Permit. Failure to caiply with the foregoing <br />may result in -rescission of the Permit, and is in violation of Chapter 112.89 MSA, a mis- <br />demeanor. <br />This document is not transferable, and is valid for one (1) year after the date of issuance. <br />No construction is authorized beyond expiration date. If Permi.ttee requires more time to <br />complete construction, a renewal of the Permit will be necessary. <br />Please contact us at 483-0634 when the project is about to oonuence so an inspector may view <br />the work in progress. Permittee has paid for 2 inspections: one at project commencement and <br />one at oompletion. Any additional inspection requested or required will be billed at $15/visit. <br />By acceptance of this permit,the permittee has consented to the foregoing. The enclosed Owner's <br />Certification must be forwarded within ten days of project oonpletion. <br />Date of Issue: December 5, 1988 <br />12 <br />
