ATTACHMENT 4 <br />92 A.Form. Any person or organization desiring to hold a Temporary Cannabis Event must <br />93 apply for a permit using the application provided. Incomplete applications will be <br />94 returned to the applicant with details on how to make the application complete. In <br />95 addition to other relevant information, the application must contain the following: <br />96 <br />97 1.Applicant name, address, phone number; <br />98 <br />99 2.Address of proposed Temporary Cannabis Event; <br />100 <br />101 3.Name of property owner, if different from applicant, and signature of property <br />102 owner authorizing use of property for the Temporary Cannabis Event; <br />103 <br />104 4.A copy of the application for a Temporary Cannabis Event Organizer license <br />105 submitted to the Office of Cannabis Management under, and meeting the <br />106 requirements of, Minnesota Statute Section 342.39, subd. 2; <br />107 <br />108 5.A diagram of the Temporary Cannabis Event showing: <br />109 <br />110 a.Location and description of sanitary facilities meeting federal and state <br />111 requirements; <br />112 <br />113 b.Location and description of solid waste disposal facilities meeting state and <br />114 local regulations; <br />115 <br />116 c.Location and description of mobile food vending to be offered at the <br />117 Temporary Cannabis Event; proof of license and permit for vending must be <br />118 submitted at least seven days prior the event and kept on site for immediate <br />119 inspection. <br />120 <br />121 6.Security: In addition to meeting the requirements of Minnesota Statute Section <br />122 342.40, subd. 3, the City may require the applicant employ, at their own expense, <br />123 additional security personnel necessary to protect maximum number of persons <br />124 permitted to attend the event and to preserve order in and around event site as <br />125 determined by the City. No permit shall be issued unless the City police and fire <br />126 departments have approved the security plan. <br />127 <br />128 7.Emergency Plan: Applicant shall provide the City with an emergency plan that <br />129 details procedures for managing or responding to emergencies as required by the <br />130 Minnesota State Fire Code. In addition, for events where there is a possibility for <br />131 more than 1,000 people to congregate, the applicant shall provide trained crowd <br />132 managers. The minimum number of crowd managers shall be established at a <br />133 ratio of one crowd manager to every 250 persons. Where approved by the fire <br />134 code official, the ratio of crowd managers may be permitted to be reduced only <br />135 where the facility is fully equipped with an approved automatic sprinkler system <br />136 or based upon the nature of the event.
