<br />Dated: <br /> <br />NORTH SUBURBAN FRANCHISE COMPLIANCE EVALUATION <br /> <br />The following are requirement$ of the North Suburban Franchise <br />Ordinance. Please review these requirements and explain in detail <br />under the "Compliance" column your compliance with these requirements. <br />Should you require more space for your response, either expand the <br />forms or reference attached material. If you believe your compliance <br />has been modified or excused by franchise amendments or other agreements <br />with the Commission or its member cities, please explain with specificity, <br />referencing and quoting the official document which modifies or excuses <br />your obligations. <br /> <br />Note: Any failure to list a franchise requirement shall in <br />no way excuse your existing obligation to comply.
