(� Annual Statement of Benefits. On or before <br />January 31 of each calendar year, the Employer shall furnish to <br />each Participant who received benefits under paragraph 5.2.3 <br />during the preceding calendar year, a statement of all such benefits <br />paid to or on behalf of the Participant during the prior calendar <br />year. <br />(g) Separate Written Plan. For purposes of the Code, <br />paragraph 5.2.3 shall constitute a separate written plan providing a <br />program of dependent care assistance. To the extent necessary, <br />other provisions of the Plan are deemed incorporated by reference <br />in paragraph 5.2.3. <br />5.2.4 Dental Covera�e. Payment of the employee cost of coverage under <br />the City of Roseville Dental Plan and under such policies or programs as the <br />Employer elects to make available to the Participant. <br />5.2.5 Life Insurance Covera�e. Payment of the employee's share of the <br />cost of coverage under the City of Roseville Life Insurance Plan for a benefit <br />amount not to exceed $50,000. <br />5.2.6 Health Savin�s Accounts. An additional option available to be offered <br />under the Plan is that of a Health Savings Account. <br />(a) HSA Eli�ibility.
