upon the presentation to the Employer of documentation of these <br />expenses in a form prescribed by the Employer. However, in its <br />discretion, the Employer may pay any of these expenses directly, in <br />which event it shall be relieved of all further responsibility with <br />respect to that particular expense. These expenses shall be paid <br />periodically during the Plan Year upon receipt of a claim <br />complying with Plan requirements, and no later than March 31st <br />following the close of the Plan Year upon receipt of a claim (no <br />minimum) complying with Plan requirements. <br />(e) Forfeiture of Unused Benefits. If, following the <br />final payment of reimbursement benefits for eligible expenses <br />incurred during the Period of Coverage for any Plan Year, any <br />amount remains in a Participant's Dependent Care Reimbursement <br />Account for that Plan Year, the Participant shall forfeit such <br />amount to the Employer, and shall have no further claim to that <br />amount. If not retained by the Employer, forfeitures may be used <br />in one or more of the following ways: to reduce the required salary <br />reduction amounts for the immediately following plan year on a <br />reasonable and uniform basis; returned to the Participants on a <br />reasonable and uniform basis; or, to defray expenses to administer <br />the plan. <br />38 City of Roseville MPD <br />
