For the Medical Expense Reimbursement Plan these events are: A change in a <br />participant's legal marital status due to marriage, divorce, death of a spouse, legal <br />separation or annulment; A change in the number of the participant's dependents because <br />of birth, adoption, placement for adoption, or death; A change in the employment status <br />of a participant or the participant's spouse or dependent(s) such as termination or <br />commencement of employment, strike or lockout, commencement of, or return from, an <br />unpaid leave of absence, or a change in work site. If the participant, the participant's <br />spouse or the participants dependent has a change in employment status that affects <br />eligibility under an employer plan; Events that cause a dependent to satisfy or cease to <br />satisfy eligibility requirements of an employer plan; A judgment, decree, or order <br />("order") resulting from a divorce, legal separation, annulment, or change in legal custody <br />(including a qualified medical child support order) which requires accident or health <br />coverage for a participant's child (including a qualified medical child support order); or <br />commencement or return from FMLA leave. A Participant is not permitted to change an <br />election to the Medical Expense Reimbursement Plan as a result of a cost or coverage <br />change to a benefit plan. <br />For the Dependent Care Plan these events are: A change in a participant's legal <br />marital status due to marriage, divorce, death of a spouse, legal separation or annulment; <br />A change in the number of the participant's dependents because of birth, adoption, <br />placement for adoption, or death; A change in the employment status of a participant or <br />the participant's spouse or dependent(s) such as termination or commencement of <br />12 City of Roseville MPD <br />
