<br />This permit is not assignable by the permittee except with the written consent of the Coaunissioner of Natural Resources.
<br />The perntittee shall notify the Area Hydrologi>< at hart five days in advance of the commencement of the work authorized hereunder and notify him/her of its completion within
<br />five days. The Notice of Permit issued by thz Commissioner shall be kept securely posted in a conspicuous place at the siu of operations.
<br />The permittee shall makz no changes, without writtett permission previously obtained from the Commissioner of Natural Resources, in the dimensions, capacity, or location
<br />of any items of work authorized hereunder.
<br />The pernuttee shall grant access to the site at all reasonable times during and after construction to authorized repres.°ntatives of the Commissioner of Natural Resources for
<br />inspection of [he work authorized hereunder.
<br />This Permit may be temtinated by the Comrissioner of Natural Resources at any time deemed necessary for the conservation of water resources of the state, or in the interest
<br />of public health and welfare, or for violation of any of the provisions of this permit, unless otherwise provided in the Special Provisions.
<br />COfU2rllCtlCn work authorized under this perrnit shall be completed on or before date specified above. The pernilttee may request an zxtension of time to complete the Protect,
<br />stating the reason thereof, upon writtrn request to the Commissioner of Natural Resources.
