Attachment A Agreement between the North Suburban Access Corporation and the City of Arden Hills for Cable Services Agreement: This agreement outlines the services to be provided by the North Suburban Access Corporation (NSAC) to the City of Arden Hills related to cable services. Explanation of Services to be Provided: The North Suburban Access Corporation agrees to provide cable services to the City of Arden Hills. This will include video recording the monthly Planning Commission and City Council Meetings, and scheduling the playback for these meetings. The City of Arden Hills agrees to provide the cable technician with information/text for the bulletin board in a timely fashion. It is estimated that the total time required in Arden Hills will be approximately 13 hours per month. The North Suburban Access Corporation will provide the City of Arden Hills with technical feedback about the operations of the equipment. If the cable equipment is not functioning properly, the North Suburban Access Corporation technician will inform the Arden Hills staff of the problem to ensure that the problem is corrected in a timely fashion. The City of Arden Hills agrees to provide the equipment needed for the above services. The North Suburban Access Corporation will bill any special projects or requests for cable consultation services not outlined in the above agreement separately. The North Suburban Access Corporation will provide the City of Arden Hills with a cost proposal for any special projects or consulting activities that are requested by the City of Arden Hills but not outlined in this agreement. The requested project will not proceed unless both parties are agreeable to the cost proposal. The City of Arden Hills also agrees to provide the equipment needed for any special projects or requests for cable consultation services. Employment: The cable technician will remain an employee of the North Suburban Access Corporation during the scope of their work with the City of Arden Hills. The North Suburban Access Corporation will be responsible for worker' s compensation, salary, training and other benefits related to this position. Cost and Duration: Beginning November 1, 2013, through December 31, 2014, the City of Arden Hills agrees to compensate North Suburban Access Corporation $234/month for approximately 13 hours per month plus 10% administration fees. Additional services provided over 13 hours per month as directly related to items in paragraph 2 will be billed The North Suburban Access Corporation will invoice the City at a flat rate of $40 per hour . of Arden Hills on a monthly basis. Payment will be made within 10 working days of receipt of invoice.
