<br />Page 6 of 9 <br /> <br />3. Contractor Team Organization <br />State in writing and provide an organizational chart showing the team members and key <br />personnel. Identify the primary staff person who will serve as the project manager for <br />the duration of the contract and the staff who will likely be assigned to the work; <br />provide resumes for project manager and key support personnel. If relevant, provide <br />names of subcontractors. Include the following information for each proposed <br />subcontractor: <br />a) Identification of subcontractors, their areas of expertise, and a description of <br />how they will be used in this project; <br />b) Provide no more than a one (1) page resume for each proposed sub‐contractor <br />staff person assigned to this proposal/contract. <br />c) List up to one relevant project completed in the past five (5) years by each <br />subcontractor assigned to this proposal/contract. <br /> <br />4. Statement of Understanding <br />Include a statement of understanding in the proposal that demonstrates that the <br />Proposer fully understands the purpose, goals, and scope of the project as described in <br />this RFP. <br /> <br />5. Work Plan <br />The Proposer shall provide a work plan which includes the following: <br /> <br />a) A narrative describing how the Proposer intends to accomplish the scope of <br />services and produce the deliverables described in this RFP, including <br />i. How your firm intends to deliver the required services; <br />ii. A description of the specific steps to be taken to carry out the work; <br />iii. A Workflow diagram ; <br />iv. A timeline for the work, including key milestones; <br />v. Additional tasks or requirements not reflected in the “Scope of <br />Services” that the Proposer deems essential for successful <br />completion of the project. If the “Scope of Services” appears <br />sufficient, so note in your response. <br />b) A narrative on any innovations and or novel approaches that the Proposer <br />envisions using for the project. <br /> <br />5. Budget <br />The contractor shall include a project budget divided into Phases and Tasks as <br />outlined in the work plan. In no event shall the payment for services associated <br />with this project exceed $200,000. [Needs Department of Finance input] <br /> <br />6.
