AnY person desirinq a small kennel license <br /> reauired under the provisions of Section 5-67 shall make written <br /> application therefor to the City Administrator upon a form <br /> prescribed by and containina such information as required bv the <br /> City Administrator. A descriPtion of the real property upon which it is <br /> desired to keep the animal or animals. <br /> III The species and number of animals to be maintained on the <br /> premises. <br /> ill A statement that the applicant/permittee will at all <br /> times keep the animals in accordance with all the <br /> conditions prescribed bv the City Health Officer and/or <br /> Animal Control Officer, or a modification thereof, and <br /> that failure to obey such conditions will constitute a <br /> violation of the proyision of this chapter and qrounds <br /> for cancellation of the permit. <br />. Jtl Such other and further information as may be required by <br /> the City Health Officer and/or Animal Control Officer. <br /> J..Ql Consent. The applicant for any permit required under the <br /> provisions of Section 7-67 shall provide with the application the <br /> written consent of seventy-five (75) percent of the owners or <br /> occupants of privately or publici y owner real estate within one <br /> hundred fifty (150) feet of the outer boundaries of the premises <br /> for which the permit is beinq reauested or, in the alternative, <br /> proof that applicant's property lines are one hundred fifty (150) <br /> feet or more from that structure. However, where a street <br /> separates the premises for which the permit is beina reauested from <br /> other neiqhborina property, no consent is reauired from the owners <br /> or occupants of property located on the opposite side of the <br /> street. Where a property within one hundred fifty (150 ) feet <br /> consists of a multiple dwellinq, the applicant need obtain only the <br /> written consent of the owner or manaaer or other person in charae <br /> of the buildina. <br /> ltl Investiqation by the City Health Officer and/or Animal <br /> Control Officer; may qrant permit.
