<br /> I <br /> I 3. . Meetings The Accident Review Board will meet the first Monday following the <br /> report of an accident or injury. The facts of the accident will be presented <br /> It by the supervisor to whom the accident was reported. The employee <br /> involved in the accident may attend the review and present their account of <br /> the accident. <br /> I 4. Findings Within five days, the Review Board will make one of two findings: <br /> I I. Non-Preventable - With no recommendation for corrective action. <br /> 2. Preventable - With recommendation for corrective action. <br /> I 5. Corrective Action Will be consistent with the disciplinary actions outlined in the <br /> City's personal policy and may include, but is not limited to: <br /> I I. Positive Discipline - Specialized training, refresher training, formal education, <br /> I required attendance at seminar, field training program conducted by supervisor. <br /> 2. Negative Discipline - Verbal reprimand, suspension without pay, demotion, or in <br /> I extreme cases, discharge. <br /> 6. Disciplinary Action Any disciplinary action will be initiated by the employee's <br /> .. supervisor, department head, and/or city administrator, whichever <br /> is applicable. <br /> . 7. A copy of the review boards fmdings will be sent to: <br /> I. The Employee(s) involved <br /> I 2. The Supervisor to whom the accident was reported <br /> 3. The Department Head <br /> 4.
