<br />. <br /> <br />. <br /> <br />. <br /> <br />City of Arden Hills Personnel Policy <br /> <br />Effective June 29, 2002 <br /> <br />REPORTING ACCIDENTS <br /> <br />Employees are expected to adhere to all safety requirements. \XThenever an employee is <br />injured on the job, he/she shall report it to their supervisor at once. A "First Report of <br />Injury" report must be promptly flied and sent to the City Administrator giving full details of <br />the incident. <br /> <br />WORKERS COMPENSATION REPORTING <br /> <br />Workers Compensation benefits and OSHA Gob safety laws) require that all on the job <br />accidents, sickness, or loss of consciousness be reported as soon as possible by the employee, <br />or on behalf of a sick or injured employee, to his or her Supervisor. <br /> <br />EMPLOYEES MUST REPORT TO THEIR SUPERVISOR ALL JOB ACCIDENTS, <br />SICKNESS, OR LOSS OF CONSCIOUSNESS AS SOON AS POSSIBLE. <br /> <br />Where items of safety equipment are required by federal, state, or local rules and regulations, <br />it shall be a condition of employment that the employee wears such equipment. <br /> <br />If you are involved in an accident, remember: <br /> <br />1. <br /> <br />All accidents involving city vehicles should be reported to the City <br />Administrator's office as soon as possible. <br /> <br />2. Insist that all parties and property concerned remain at the scene of the <br />accident. <br /> <br />3. Obtain identifying data from the driver of the other vehicle (name, address, and <br />insurance company). <br /> <br />4. Secure names and addresses of injured persons and any witnesses to the <br />accident. <br /> <br />5. All accidents and/ or injuries must be reported on a form supplied by the City <br />and should be returned as soon as possible from the time of the accident. <br /> <br />37 <br />
