\ Attachment to Change Order No.1 . Owner: Contractor: Project: City of Ardcn Hills Amt Constrnction Company, Inc. 2006 Pavement Management Program Description of changes: Several modifications are proposed to the project plans in ordcr to bring project costs closer to available funding, The modifications are included with the Roadway, Storm Drainage and Watermain portions of the project. As a result ofthcsc modifications, the scope of the project will decrease, as well as the contract amount for the project. The following is an overview of the modifications proposed to the contract: Schedule A - Roadway The common excavation quantity included with the bid is based upon a roadway cross section which includes 8" of aggregate base and 4" of bituminous. The final design was modified to include 6" of aggregate base and 3,5" of bituminous. The common excavation quantity shall be reduced to reflect the thinner pavement section as follows: I, Reduce common excavation by 910 CY @ $12.34 ICY = 2. Reduce Bulkhead Pipe by 2 Ea @ $S20.00/Ea = Subtotal Schedule A - Roadway ($11,22940) ($1.040,00) ($12,269.40) . Schedule B - Storm Drainage The storm sewer scope is proposed to be reduced by utihzing an existing outlet to Lake Johanna thereby eliminating the new outlet included witli the bid, In addition several miscellaneous modifications to the storm sewer pipe alignments and sizes are proposed throughout the neighborhood to reducc costs, Thc proposed changes will not adversely effect the effluent watcr quality for system discharges into Lake Johanna. The proposed modificatIOns include pipc and grading changes requested by thc DNR and Rice Creek Watershed District as a part of permit approvals, The proposed modifications are outlined below: I, Increase Common Channel Excavation by 578 CY @ $14.S8/CY = $8,427.24 2, Reduce 29"XI8" RC Pipe Apron by 3 Ea @ $I,460.00/Ea = ($4,380.00) 3. Reduce 29" X18" Trash Guard by 2 Ea@ $S06,00/Ea = (SI,012,OO) 4. Reduce 12" RC Pipe by 200 LF @ $34,00/LF = (S6,800,00) 5, Reduce 29"X 18" RC Arch pipc by III LF@ $78.00/LF = (58,658,00) 6. Reduce 18" HDPE Pipc Sewer (Bored) by 230 LF @ $IS4,00/LF =($35,420.00) 7. Increase 30" RC Pipe by 128 LF @ $63,00/LF = $8,064.00 8, Reduce 48" Drainage Structure by 10 LF @ 5250.00/LF = ($2,500.00) 9. Reduce Floating Silt Curlain by 100 LF@$12.00/LF= ($1,200,00) 10.
