<br />. <br /> <br />. <br /> <br />. <br /> <br />Blue Cross and Blue Shield of Minnesota <br />3535 Blue Cross Road, Eagan, Minnesota 55122 <br />Contractor Agreement <br /> <br />retained by BCBSM as technical assistance provider ("TA provider"), A <br />proposed schedule of educational sessions is attached hereto as Appendix C, <br />which by this reference becomes a part of this Agreement. <br /> <br />3, Contractor will consult with the T A provider to finalize contractor's schedule of <br />participation, <br /> <br />4, Contractor will prepare and submit a schedule of activities and meetings related <br />to the completion of Comprehensive Plan updating to the T A provider and <br />BCBSM, <br /> <br />5, Contractor will submit a draft of the Comprehensive Plan to the T A provider for <br />preliminary review and comment. The timing of draft submission will be jointly <br />determined by the TA provider and Contractor, <br /> <br />6, Contractor agrees to consult with the T A provider during the course of the <br />Comprehensive Plan updating process, with the draft Plan being subjected to <br />review, and consideration as a potential model for other municipalities, Such a <br />request will be made at the discretion of the T A provider and BCBSMN <br />subsequent to the preliminary review referenced in paragraph 5 above, <br /> <br />7, Contractor will employ the Health Impact Assessment tool provided by the TA <br />provider. <br /> <br />8, Contractor will (if located within the seven-county Twin Cites Metropolitan Area) <br />submit the resulting Comprehensive Plan to the Metropolitan Council for review <br />and approval consistent with schedules established by the Metropolitan Council <br />for their mandatory updating of Comprehensive Plans, <br /> <br />9, Contractor will submit a copy of the final Comprehensive Plan revision to both <br />the TA provider and BCBSM, <br /> <br />10, Contractor will designate one (1) staff person responsible for coordinating their <br />funded activities and serving as primary contact for BCBSM and the T A provider, <br /> <br />11, Contractor will provide BCBSM Project Manager the opportunity to review before <br />distributing and/or implementing letters to the editor, press releases (including <br />joint press releases), and advertising that references BCBSM, <br /> <br />12, Contractor will provide BCBSM project manager with copies of all media <br />coverage, <br /> <br />13, Contractor will make available to BCBSM copies of any products produced by <br />activities supported by these funds, <br /> <br />Page 2 of 6 <br />
