APPENDIX B <br /> INSURANCE <br /> The EMPLOYER will pay a flat fee of one hundred dollars ($100) <br /> per month for permanent part -time employees towards single health <br /> insurance. Any premium amount over the $100 will be paid by the <br /> employee. If insurance is not elected or the health insurance <br /> premium is less than $100, the employee may use the allocation <br /> towards additional life insurance or taken as deferred <br /> compensation. <br /> The EMPLOYER will pay a flat fee of one hundred fifty dollars <br /> ($150) per month for permanent full -time employees towards single <br /> health insurance. Any premium amount over $150 will be paid by <br /> employee. If insurance is not elected or the health insurance <br /> premium is less than $150, the employee may use the allocation <br /> towards additional life insurance or taken as deferred <br /> compensation. <br /> The EMPLOYER will pay a flat fee of three hundred dollars ($300) <br /> per month for permanent full -time employees towards family <br /> insurance coverage. Any premium amount over $300 will be paid by <br /> employee. To be eligible for the family coverage the employee <br /> must provide coverage to a minimum of two persons from the <br /> immediate family. If the premium is less than $300, the employee <br /> may use the allocation towards additional life insurance or taken <br /> as deferred compensation. <br />
