CITY OF ARDEN HILLS - ENGINEERING SERVICES Request for Proposals EXHIBIT "A" PROPOSAL CONTENT SU BMISSION RE 1RFMENTS At a minimum, your proposals shall include the following irrmation which will be used in the evaluation and selection process: GENERAL INFORMATION FIRM BACKGROUND _. _- In this section, present ahy information about your firm th you feel is appropriate. However, include the size of firm, year firm as established, number of s taff at this time, and an organizational chart by job cat Qbry. EXPERIENCE Present the areas of specialization for which your firm is recognized, and any key features that might set your firm apart. Include names of principals and key personnel with experience of each, years of membership in the firm, and types of specialization. List municipalities in fiich your firm has been involved over the last five (5) years. - 'The qualifications, tions resurnes and a listing of recent assignment's of the professional staff that will be assigned on an ongoing basis to City projects. Please designate �u the primary ongoing contact and backup person nel�PIease expand on the abilities and experience of the construction inspector(s) who would be assigned to the CitmProvide the availability of the aforementioned staff, including all present and ongoing assignments. MUNICIPAL CLIENTS Provide a listing of previous municipal clients including projects worked on, specific activities performed and the name of a person who may be contacted at the municipality in which the project occurred (i.e., plannin zonin rastructure, Chapter 429 projects, etc.). Provide a listing of current clients projects being worked on and the name and phone number of a contact person. (We would prefer an emphasis on municipal clients.) If your firm provides both private sector and municipal engineering services, please explain in more detail.
