<br />Procedure: The procedure fot apply.irig fot light duty assjgnmei1t wiD. be as follows: <br /> <br />A) When an Employee is unable, due to temporarily disability, to PtQvide full <br />.petfonnance of all won: duties assigned to the Employee's job classification, <br />the Employee may request light duty' by .completing the Requ~ fot light <br />Duty fotnl and submitting it to his/het Department Head. The request shall <br />state the natute and extent of the temporaty disability .and the job duties <br />which the Employee is unable. to petform, along with the expected length of <br />the disability and any work: restrictions related to the disability. The City wiD. <br />. usually .requite the tequest to be accompanied .by a physician's report <br />~onrn1n1ng diagnosis, cw:renttteatment, and physician apptoval fot light duty <br />assignment with any testrictions theteon. <br /> <br />B) The City may require an independent evaluation conducted by a physician <br />selected by the City to verify the diagnosis, cuttent treatment, won: <br />restrictions and eXpected length of disability. <br /> <br />C) DetP.1'm1nqtions :regatdinglight duty wiD. be made on a depattment-by- <br />department, case-by-case basis. The Department Head, in consultation with <br />. the City Administrator, will consider such factors as public setrice needs and <br />budgetaty considerations, the need fot wotk which may be assigned as light <br />duty, the Employee's capability of petfotming the wo.rk, the numbet of <br />Employees not available for wotk due to injuty ot illness, and othet relevant <br />. factors. <br /> <br />D) The City will determine what job duties the Employee wiD. petfOtnl. These <br />duties may include those cuttently assigned to the Employee's job <br />classification or any othet duties the City considets to be appropriate. <br />Du.tation of light duty will be determined at the sole discretion of the City. <br /> <br />E) <br /> <br />Assignments to light duty will be reviewed on a monthly basis ot more often <br />as deemed necessaty by the City. The City, in its sole discretion, reserves the <br />right to tenn1n~te a light duty aslrig1tment at any time based upon, but not <br />limited to, the factors set forth in sub-item C) above. <br /> <br />11. INSURANCE PLANS <br />11..1 Health, Dental, Disability, & Life Insurance <br /> <br />Upon proper application and subject to payment of any .requited premiums, all <br />.regular full time Employees, working at least 32 hours pet week, will be :requited to <br />. be coveted by the City's health, dental, short-term disability and life insurance plans. <br />Subject to the payment of the .required premiums, an Ettiployee's dependents may <br />also be coveted under such plans. . <br /> <br />25 <br />
