<br />11.2 Cafeteria Plan <br /> <br />The City will contribute an amo~t, d-ignated on an ann~ basis by acti~n of the <br />City Council, to the Employee for use in the Cit(s Cafeteria Plan. Employees are <br />required to choose at ~ the core benefit p~ as defined in the Cit(s Cafeteria <br />Plan document. .Any balance ~g1n1ng after the required core benefits have been <br />purchased may be used in any authorized area of the plan, such as: <br /> <br />(A) The purchase of additional life; disability, or long term. carejnsurance offered <br />through the Cit(s plan; <br /> <br />(B) . Contribution to an authorized flexible spending account; <br /> <br />(C)" Contribution to. an Employer sponsored deferred compensation (457) <br />program. <br /> <br />(D) . Or, the Employee may elect to receive the ba1a.nce in taxable income. <br /> <br />ll.3 Continuation of Benefits <br /> <br />Health Plans: Employees will be allowed to continue health and dental insutance <br />coverage upon tP.1'm1ngtion in accordance with Minnesota Continuation laws and/or <br />federal COBRA regulations. Certain "qualifying" events trigger an Employee's <br />andh~r dependents' right to continuation coverage. <br /> <br />TerminAtion or Reduction: Generally, an. Employee and his or her dependents are. <br />allowed to continue coverage for a period of eighteen (18) months when one of the <br />. following qualify.ing events occuts: <br /> <br />. A reduction in the Employee's work hours (mcluding strikes or <br />layoffs); or <br />. V oluntai:y or involun~. tP.nningtion of the Employee for reasons <br />other than gross misconduct. <br /> <br />Divorce, Separation, Death: In the instance of divorce, legal separation or death <br />of the Employee, a spouse and/or dependent child may continue. coverage until: <br />· Coverage is obtained under another group plan; or <br />· Until coverage would otherwise end under the plan, such as the City <br />tMm1ngting their group coverage for all Employees. <br /> <br />Life Insurance Benefits: Generally, if an Employee loses coverage- through <br />tMmination of employment'or reduction in hours; s/he has a right to continue group <br />life insutanee benefits for a period of eighteen (18) months from the loss-of the <br />coverage date. . <br /> <br />26 <br />
