<br />CENTENNIAL FIRE DEPARTMENT RELIEF ASSOCIATION BYLAWS <br /> <br />Section 2. <br />All applications tor disability benefits shall be submitted <br />to the board of trustees at a regular or special meeting of <br />the board. The application shall be accompanied by a <br />certificate from the attending physician or surgeon setting <br />forth the nature of the illness or injury, the cause and <br />duration thereof, the length of time the applicant has been <br />unable to perform any of the duties connected with his <br />regular occupation and those of a firetighter, and an <br />estimate of the time at which the applicant will be able to <br />return to his/her regular occupation and to perform the <br />duties of a firefighter. <br /> <br />Section 3. <br />All applications for pensions shall be submitted to the <br />board of trustees at a regular or special meeting of the <br />board. Applications shall be verified by an oath of the <br />applicant and shall state the age of the applicant, the <br />period of service in and the date ot retirement from the <br />Centennial Fire Department, the length of time he/she has <br />been a member of the association, and such other information <br />as the board of trustees may require. <br /> <br />Section 4. <br />No benefits or pensions shall be paid until the application <br />therefore has been approved by a majority vote of the board <br />of trustees. Decisions ot the board shall be final as to <br />the payment of such benefits or pensions. No person <br />receiving a pension shall be paid any other benefits by this <br />association. <br /> <br />Section 5. <br />For purposes ot computing benefits payable under Articles <br />VII and VIII, a "year of service" shall be defined as a <br />period of twelve full months of active duty in the <br />Centennial Fire Department, beginning on the anniversary of <br />the date when the member became an active member of said <br />Fire Department. If a member's period of active service lS <br />not continuous, parts of years may be added together to <br />compute full years.
