<br />e. Length of Leave. Family/Medical Leave shall <br />be taken simultaneously with the Minnesota <br />Parenting Leave. The length of FMLA leave is <br />not to exceed twelve (12) weeks in any twelve <br />(12) month period. The entitlement of FMLA <br />leave for the birth or placement of a child <br />expires twelve (12) months after the birth or <br />placement of the child. <br /> <br />f. <br /> <br />Leave Year. <br />first day the <br />FMLA leave. <br /> <br />The leave year will begin the <br />employee is absent from work <br /> <br />on <br /> <br />g. Notice. Employees must give the City at <br />least 30 days advance notice if the leave is <br />foreseeable. If leave must be taken in less <br />than 3 days, the employee should give as much <br />notice as is practicable. <br /> <br />h. Medical Certification. Employees must <br />provide a medical certification if the <br />employee will be using unpaid leave and the <br />leave is for the serious health condition of <br />a child, spouse, parent or the employee. A <br />"Certificate of Physician of Practitioner" <br />form can be obtained from the <br />Payroll/Benefits Coordinator. It is to be <br />completed by the attending physician or <br />practitioner. A "Certificate of Physician or <br />Practitioner" is not required if the employee <br />will be using paid leave for the duration of <br />the FMLA leave, or the employee is on <br />workers' compensation. <br /> <br />The City may require a second or third <br />medical opinion at the City's expense. <br /> <br />i. Use of Accrued Sick Leave, Vacation Leave & <br />Compensatory Time. Employees may choose to <br />use accrued sick leave, vacation leave and/or <br />compensatory time while on FMLA leave, but <br />will not be required to do so by the City. <br /> <br />The use of sick leave, vacation leave and/or <br />compensatory time occurs simultaneously with <br />FMLA leave and does not extend the length of <br />FMLA leave. <br /> <br />V-6 <br />
