<br />employee has an objective basis in fact to believe <br />violates any state or federal law or rule or <br />regulation adopted pursuant to law, and the <br />employee informs the supervisor that the order is <br />being refused for that reason. Notwithstanding <br />the above, the City reserves the right to take <br />action against an employee who makes statements or <br />disclosures knowing that they are false or that <br />they are in reckless disregard of the truth. <br /> <br />2. <br /> <br />Release of Protected Information. <br />pri vate or protected information <br />Minn. The City Administrator or the <br />City Administrator's designee shall maintain <br />personnel files. The files are not available to <br />the public except as provided by law. The City <br />Administrator, City Attorney, and an employee's <br />department head are authorized to view an <br />employee's file at any time. Upon written request <br />by an employee to the employee's department head, <br />the City shall provide the employee with an <br />opportunity to review the employee's personnel <br />record, as provided by Minn. Stat. Sec. 181.960 et <br />seq. Unauthorized viewing, removal, alteration, <br />or destruction of all or any part of an employee's <br />personnel file is prohibited. Each personnel file <br />shall include a page noting changes made and dates <br />of the changes. <br /> <br />H. Safety. <br /> <br />1. In General. Employees must take proper <br />precautions to prevent accidents and work safely. <br />Employees shall follow safe practices and all <br />safety regulations concerning their job. Injuries <br />should be reported immediately to the employee's <br />supervisor. Any death or serious injury must be <br />reported on the First Report of Injury form, which <br />is available from the payroll clerk. The form is <br />required each time an injury results in an <br />employee being unable to work. Any employee who <br />notices an unsafe condition is encouraged to <br />immediately report it to the employee's <br />supervisor. <br /> <br />VI-20 <br />
