<br />b) Any employee covered by Worker's Compensation who is injured <br />on the job and is subsequently off work for three (3) consecutive <br />days because of the injury will earn worker's Compensation <br />benefits. <br /> <br />c) All work-related injuries must be reported to the employee's <br />supervisor immediately, no matter how minor the injury is <br />considered by the employee. <br /> <br />12.2 Health Insurance <br /> <br />Upon proper application and subject to any required premiums, all regular <br />full time employees may elect-to be covered by the City's health insurance <br />plans. Subject to the payment of the required premiums, an employee's <br />dependents may also be covered under such plans. <br /> <br />a) The City Council will periodically review the percentage <br />of premiums the City pays and will determine whether a <br />change in the amount is warranted. <br /> <br />b) The balance of the monthly premium will be paid by the <br />employee through authorized payroll deductions. <br /> <br />c) Any employee leaving City service for any reason may <br />continue coverage under the City's group insurance <br />plan for a period not to exceed Federal or State <br />requirements. Such employees must pay the entire <br />premium for that coverage and may not upgrade the <br />coverage from single to family, unless otherwise <br />required by law. <br /> <br />d) The City will pay one-half (1/2) of the cost of the City's <br />health insurance premium for all part-time employees. <br /> <br />12.3 Dental Insurance <br /> <br />Upon proper application and subject to any required premiums, all regular <br />full-time employees may elect to be covered by the City's dental insurance <br />plan.
