<br />, <br />, <br /> <br />" <br /> <br />7.0 SUBMISSION REQUIREMENTS <br /> <br />Each submission shall be presented in an 8 'h" x 11" vertical, bound format. Materials not specifically requested <br />(brochures or promotional items) will not be considered in the selection process. The submissions shall be limited to <br />twenty (20) pages or less. Ten (10) copies of the submission shall be submitted at the time indicated in the following <br />proposal schedule section. Submit one master copy and nine photocopies (black and white) of the complete package. <br />Submittals become the property of the City of Centerville and will not be retumed. <br /> <br />All submissions must be received by the City of Centerville by Thursday, June 16, 2005 at 4:00 p.m. Submissions shall <br />be addressed to John Meyer, Finance Director, City of Centerville, 1880 Main Street, Centerville, MN 55308. All phone <br />calls regarding this RFP shall be directed to Mr. Meyer at 651-429-3232. No verbal directions will be given. All <br />questions regarding this project must be submitted in writing to Mr. Meyer by the deadline indicated. Questions and <br />responses will be forwarded to all invited finns. <br /> <br />Specific Requirements: Each submission must be arranged according to the following outline: <br />1. Title Sheet <br />The title sheet must provide the name(s) of the Firm submitting with the name of the prime contact clearly <br />identified. <br />2. Cover Letter <br />The cover letter must indicate the composition of the entire Firm including the Principal in Charge, Project <br />Manager and support team. Include primary contact name and phone number. <br />3. Table of Contents <br />4. Background information for the firm <br />Summarize the firm, its background and focus. List and describe the principals of the firm, their length of <br />association and general background. Provide a list of current active projects including the name, type of <br />project, location, firm's role in the project and status of the project. <br />5. Relevant experience of the firm <br />List three to five (3 to 5) examples of planning/design projects comparable in scale, program, process, <br />component parts and function to this master plan project. Planned as well as built projects are eligible. <br />Include drawings, photographs or published materials to illustrate the nature and scope of each example <br />project. For all projects listed as relevant experience, include the following: <br />. Location <br />. Client <br />. Date of project involvement <br />. Project description including illustrative materials and a written description of why this example is <br />relevant to this master planning project <br />. Cost and scope of the project <br />. Specific services preformed by the finn. If the firm submitting this proposal was not the principal <br />firm in charge of the example project, indicate the principal finn. <br />6. Professional references <br />List a minimum of three (3) references for the firm. List references for the example projects. For each <br />reference list the contact's name, address, phone number and relationship to the firm. <br />7. Statement of philosophy and project understanding <br />Provide a concise statement (one page) outlining the philosophy of the firm in approaching projects of this <br />nature and the firm's grasp of issues and goals to address in Centerville's master plan study. <br />8. Work program <br />Outline a proposed work program, providing descriptions: of work tasks, key individuals on the firm who <br />will perform the tasks, work product, and schedule for completion. <br />9. Overall project schedule <br />Provide a proposed timeline for completing the scope of services. Identify project milestones that will be <br />reached during the master plan study. <br />10. Project budget <br />Present a proposed budget to complete the master plan study. The budget should be task-based (related <br />to the proposed work program) and present fully loaded labor cost, by firm member. The budget should <br />also include an itemization of anticipated expenses. The budget section should include a description of <br />approaches that will be used to contain cost and ensure quality control. <br /> <br />Request for Proposal <br /> <br />Page 5 <br />
