4� <br />CITY OF GEM LAKE <br />NPDES - MS4 SWPPP <br />BMP DESCRIPTION SHEET <br />A- Unique Identification Number: 3-03-R <br />B. Name or Type of BMP: Storm System Outfall and BMP Map <br />C. BMP Description: Preparation of a map that indicates the location of appropriate storm sewer system components and receiving water bodies. <br />D. Minimum Control Measure Addressed: Elicit Discharge Detection & Elimination <br />E Describe the measurable that will be used to determine success or benefits of this BMP: <br />1. Collect and add to the map (as needed) existing storm sewer and other structural BMP's by 10 March 2004. <br />2. Verify accuracy of draft map and standardize map nomenclature by 10 March 2004. <br />3. Update map annually to include new information collected as part of the inspection program described in BMP 6-03-R. <br />4. Complete MS4 map by 10 March 2008. <br />F. Describe the timeline or implementation schedule for this BMP: <br />1. Year 1: Identify all ponds, streams, lakes, and wetlands, structural pollution control devices (grit chambers, separators, etc.), pipes and <br />conveyances 24 inches in diameter and over, all outfalls to defined waters. discharge points leaving the MS4 and structures that discharge <br />directly to the ground water. <br />2. Year 1 to Year 5: coordinate mapping updates with streets and storm system maintenance staff and with engineering/planning staff involved <br />in review and implementation of new practices. <br />3. Year 5: completed MS4 map incorporated into final annual report due on 10 March 2008. <br />G. Person or department in charge of implementing this BMP: <br />1. Frederic C. In 2003 all structural pollution control devices (grit chambers, separators, etc.), pipes and conveyances 24 inches in diameter and <br />over. all outtalls to defined waters, discharge points leaving the MS4 and structures that discharge directly to ground water were <br />identified to the City Iingincer for incorporation with data already in place regarding identification of all ponds, streams, lakes and <br />wetlands. A new preliminary map is currently in production on or before 10 March 2004. <br />2. 2004 <br />a. A hand written map of all structural pollution control devices, pipes and conveyances 24 inches in diameter and over, all outfalls to <br />defined waters, discharge points leaving the MS4 and structures that discharge directly to ground water has been completed and will <br />be sent to the City Engineer for electronic development and updating. <br />11 <br />
